Patient Care Policies
As our patient, we want to familiarize you with some of our policies and practices relating to patient care. We hope you will find the information helpful in answering some of the questions you might have and in promoting a positive experience at our office.
Prescription Refills:
If you call for a prescription refill, please be prepared to provide the following details: your name, date of birth, prescription(s) being requested, name and phone number to the pharmacy you would like us to use. Please make your request during regular office hours (8-5 Monday-Thursday and 8-12 on Fridays) and allow up to 48 hours for them to be called in. Refill requests being called in after noon on Friday, or over the weekend, will be addressed the following week. If you have not been seen by our office within the past 6 months, you will need to schedule an appointment to be reevaluated before an additional refill will be granted.
Prescriptions:
At the time of your visit you may be provided with prescriptions to take to your pharmacy to fill. Please be advised that Dr. Palceski and his staff prescribe the medicines they feel will be the most effective in treating your condition(s). We have no affiliation with any pharmacy nor are we aware of their pricing for medicines. If you find the cost to fill your prescription(s) is too high, please contact our office so that we may initiate a prior authorization with your insurance company or provide you with a less expensive alternative. Please be advised that the prior authorization process takes up to 7 days to complete. Should you have any questions about your prescriptions please refer to the medication instruction sheet provided to you at the time of your visit or call our office for assistance.
Lab Tests:
It is sometimes necessary to order lab tests in order to obtain additional information about your condition and/ or to monitor the effects of treatment. If lab work is required prior to a return visit to our office, we do ask that you complete it at least 48 hours in advance to allow ample time for the lab to process and send results to our office. It is your responsibility to ensure that the results of any lab testing are received by our office prior to your scheduled appointment. Results may be faxed to us at 407-291-3800 or you may bring them with you. If lab work is needed, but not completed prior to your appointment, you will be asked to reschedule your appointment.
Referring Physicians:
Coordination of care is a top priority. If you are referred to our office by another physician, please let us know so that we may promptly follow up with results from your visit. If a referral is required, please bring it with you and provide it to our intake specialist at the time of check-in.
Medications:
We ask that whenever you come in for an appointment that you bring a list of all current medications. You may want to consider keeping this list in your wallet or purse to share with other doctors as needed. Whenever you are prescribed a new medication, please read any information that might come with it and check with your pharmacist to make sure it doesn’t interfere with any of your other medicines.
Insurance:
We accept most major insurance plans. If you do not see your plan listed or should you have questions regarding your plan benefits and eligibility, feel free to call our office for assistance.

Payments:
Payment, including coinsurance, patient balances, and deductibles will be collected at the time of service. We gladly accept most forms of payment including cash, check, Visa, Master Card, and American Express.
